§1. Parties to the Agreement

1. This consent is granted by:

   hereinafter referred to as the "Participant"
   to:
   Aesthetic Medicine Academy "Wenus",
   organized by Pharma Medical House Sp. z o.o. with its registered office in Warsaw, ul. Pod Lipą 4/U6.
   hereinafter referred to as the "Organizer"

§2. Scope of Consent

1. The Participant agrees to:
   a) the free recording, processing, and publication of their image (including photographs, video recordings, excerpts from speeches and statements) in connection with participation in the “Wenus Academy Awards 2026” project and the “Gala Wenus” event;
   b) the use of their name / company name, professional achievements, and promotional and educational materials (texts, photos, graphics, logo) submitted as part of the application, nomination, or participation in the event.

§3. Purpose and Form of Publication

1. The materials covered by this consent may be used exclusively for informational, promotional, and educational purposes related to the activities of the Aesthetic Medicine Academy "Wenus", including:
   • in the Wenus 2026 Album,
   • on the Organizer's websites, social media, and communication channels,
   • in press materials, media previews, and event coverage,
   • during the Gala Wenus and accompanying events.

§4. License

1. The Participant grants the Organizer a free, non-exclusive, territorially unlimited license to use the submitted materials (texts, photos, graphics, logo) for the duration of the project: from November 1, 2025, to November 30, 2026, with the possibility of archival storage and historical presentation in the future.

2. The license includes the right to:
   • record, edit, copy, publish, and distribute the materials,
   • combine the materials with other content (e.g., in albums, presentations, videos),
   • use the materials in digital, printed, and online formats.

3. The Participant confirms that the consent and license are granted free of charge and that no financial claims arise from them.

§5. Declarations

1. The Participant declares that:
   • they hold copyright or have appropriate permissions for the provided materials,
   • they consent to the processing of their personal data and image in accordance with the purpose of the project,
   • the consent and license granted in this document do not infringe on the rights of third parties.

2. The Participant may withdraw consent for image use only in writing or via email by sending a statement to: biuro@pharmamedical.pl. Withdrawal of consent does not apply to materials already published.

§6. GDPR Information Clause
Pursuant to Article 13 of the GDPR, we inform you that:
• The data controller is Pharma Medical House Sp. z o.o.
• Data is processed for the purpose of executing the "Aesthetic Medicine Academy Wenus" project – based on Article 6(1)(a) and (f) of the GDPR.
• Data recipients may include entities cooperating in the implementation of the event.
• Data will be stored for up to 5 years or until consent is withdrawn (with the exception of already published materials).
• You have the right to access, rectify, delete, restrict processing, object, and lodge a complaint with the President of the Personal Data Protection Office (UODO).
• Providing data is voluntary, but necessary to participate in the promotional activities of the project.
